Are you a dedicated and innovative Family Medicine Physician eager to shape the future of patient care? Join our team in delivering high-quality, patient-centered care through our cutting-edge Virtual Hybrid Primary Care Model. 

This innovative approach blends the best of in-person care with the convenience of virtual visits—not to replace face-to-face interactions, but to enhance access, improve continuity, and offer patients greater flexibility for follow-ups, routine check-ins, and chronic disease management. This site is designed to support polychronic patients with complex medical needs, making chronic care coordination a central focus. Providers will work alongside an integrated care team that includes behavioral health specialists, PharmDs, physical therapists, and other support services—all onsite to support whole-person care.

In this pivotal role, you'll provide comprehensive medical care, skillfully balancing virtual consultations with in-person visits. This is not about replacing face-to-face interactions; it's about expanding options for patients, ensuring they receive flexible, effective, and continuous healthcare, meeting patients where they are.

Training and/or Experience Required
  • Board-certified in Family Medicine. 
  • Experience with virtual health platforms or a strong willingness to adapt to virtual care technologies to serve our patients effectively. 
  • Demonstrated commitment to patient-centered care and a proactive approach to workflow innovation within a collaborative team environment.
  • Must be licensed or eligible for licensure to practice medicine in Illinois
