Fall 2026 Cohort Start

The projected start date is late October/early November. Applications for our Fall 2026 start date will be posted to this website no later than early January 2026. Required supplemental application elements are listed below. Once collected, these are to be attached within the online application and will not be accepted via email.

Required Application Attachments: Applications can NOT be edited after submission. Thus, please do not submit your application until you have all the materials ready. Any incomplete application will not be considered.

Satisfy program eligibility requirements by:

August 15, 2026
Completion of an accredited PA or NP master's or doctoral program
August 31, 2026
Pass the PA or NP National board examination
September 15, 2026
Obtain state licensure as a PA or APRN
Obtain DEA licensure
Successfully meet Advocate Health pre-employment requirements
Not all tracks offered in all markets
One-page Statement of Interest in postgraduate fellowship training in the specialty you are applying
Unofficial transcripts accepted for current student applicants
Three letters of recommendation are required for application. Please upload below.
  • Physician Colleague
  • APP Colleague – Mentor/Preceptor (all candidates)
  • APP Colleague – Faculty or Program Director (new grads, no experience) or Second APP Colleague (experienced APP)
APP preceptor, mentor, etc.
NP/PA Faculty Member (required for new graduates with no work experience)
APP mentor or colleague (recommended for those with working experience)
